This novel is spread over three intense days in the life of a small makeshift maternity ward for pregnant women with the Spanish flu in Dublin in 1918. Narrated by Nurse Julia Power, the acting Ward sister on day duty. With doctors and nurses succumbing daily to the flu, Julia is alone on the ward but is sent an young volunteer from the orphanage to help with her duties. Bridie might be young and naive but she's quick and smart and Julia soon starts to look on her as an able assistant and to form a strong bond with her.

In her afterward Emma Donaghue says the 100 year anniversary of the Spanish flu in 2018 prompted her to write this book. Without the advent of the current pandemic, it would still have been a compelling account of the flu, Irish politics and practices as well as the state of medicine at the time. However, it's fascinating to compare the two pandemics, 100 years apart noting what has changed but also what hasn't. Medicine of course is much more advanced; we have oxygen, ventilators and a wide range of drugs available. In 2018 they had chloroform to help with childbirth, aspirin for fever and hot diluted whisky to treat for pneumonia. Pregnant women with the flu often gave birth prematurely and without humidicribs or ICU, few survived. With no antibiotics, and Irish women malnourished and worn down by too many previous births, maternal death rates were also high at the time and made worse by the flu. Although people were warned to cover their mouths and not to congregate in crowded places and to stay home, they would still crowd onto packed buses and go to crowded cinemas to see the latest moving pictures.

Julia is a dedicated and compassionate nurse and over the course of three days we see the many horrific situations she and Bridie have to deal with. She keeps tabs on every patient she loses and grieves for each of them. We also get to meet each of her patients, some she will lose and some who will suffer much pain or heartache. Bridie’s story and upbringing as a motherless child is also shocking and heartbreaking. Dr Kathleen Lynn, an Irish Sinn Féin rebel who is filling at the hospital in for medical staff struck down with flu is based on a real person whose story makes for fascinating reading. Emma Donaghue’s extensive research into childbirth, the Spanish flu and the treatment of orphans and the impoverished of the time is impeccable and adds another dimension to the novel.
